Name Change Policy/Form

Is your legal name going to change, or have you changed your legal name? Does your BOC profile reflect your current legal name information? If not, and you want it to, complete a Name Change Request Form and submit it to the BOC.

In addition to the Name Change Request Form, submit a copy of the legal document which indicates reason for change of legal name (e.g., marriage license, divorce decree). This document must be notarized or contain the official raised seal of the department issuing the document (i.e., Department of Vital Statistics) or a stamp that clearly is an original and not a photocopy.
